Collection Counters To Be Set Up For Collecting Forms Of Cashless Health Insurance Scheme

Chandigarh, December 1, 2015: The Punjab government has set up collection counters at different places to facilitate government employees and pensioners submitting forms under the newly launched Cashless Health Insurance Scheme for the state government employees and pensioners (PGEPHIS). The Punjab government has also issued directions to all the Drawing and Disbursing Officers (DDOs) in the state to submit forms of the employees and pensioners to the Insurance Company as per the schedule released by the state government.
Disclosing this here today an official spokesman of the government said that the last date of submitting of enrolment forms is December 15, 2015. Every employee/pensioner has to submit their duly filled forms or printout of the online submitted form through their respective Drawing and Disbursing Officer (DDO). The DDOs have been asked to present the forms to the Insurance Company so that the other formalities could be completed in time.
Releasing the schedule and the place for collection of the enrolment forms from DDOs by Oriental Insurance Company, the spokesman said that a special counter would be set up by the Insurance Company in Main Secretariat where employees from the main secretariat, Vidhan Sabha and Punjab and Haryana High Court could submit their forms. Similar counter would be set up in Mini Secretariat to facilitate the employees working in Mini Secretariat.
He further said that DDOs of the different directorates have been directed to collect the forms from the counter set up by the Oriental Insurance Company near Khukrain Bhawan in Sector 35 Chandigarh. Besides this special counters would be set up in all the Suvidha Kendras at district levels so that the employees from the government offices could submit their forms without any difficulty.
The DDOs have also been directed to obtain the receipt from the Insurance Company while submitting the forms to the Company.
