Nawanshahr, September 3, 2023: A patient with bleeding hemorrhoids and complex cardiac history was treated successfully at Ivy Hospital, Nawanshahr recently.

The medical team led by Dr. Sandeep general surgeon,  Dr. Hitesh Gurjar cardiologist and Dr. Sameer Sharma anesthesiologist collaborated seamlessly to ensure the patient's safety and successful treatment.

The patient, who had a history of hypertension (HTN), dilated cardiomyopathy (DCMP) with an ejection fraction (EF) of 20%, and moderate mitral and aortic regurgitation (MR/AR), was also under blood thinners, presenting a high cardiac risk for surgery. The initial blood pressure reading stood at 100/60, making the procedure even more challenging.

To address this critical situation, the medical team decided to proceed with surgery under high cardiac risk conditions.

The patient was expertly managed to achieve a negative fluid balance prior to the surgery. The surgical procedure was carried out under plain lumbar epidural anesthesia to minimize risks. To provide hemodynamic support, a mild dose of noradrenaline (norepinephrine) was administered to maintain blood pressure.
Under the leadership of Dr. Sandeep the surgical team successfully completed the procedure to address the bleeding hemorrhoids. The patient was subsequently transferred to the Intensive Care Unit (ICU) for post-operative monitoring and observation.

Dr. Hitesh Gurjar closely monitored the patient's cardiac parameters throughout the surgical process and post-operatively to ensure cardiovascular stability. Dr. Sameer Sharma played a pivotal role in managing the patient's anesthesia, pain control, and overall well-being during the surgery and initial recovery phase.

The patient is also having bilateral lower limb numbness and cyanosis which was diagnosed and managed. Post operatively the patient didn't have any complaint with regards to the haemorrhoid surgery.
